Device Liability and Insurance Form

The Lehighton Area School District is proud to be able to participate in a 1-to-1 program in which each student is issued a district device to use while in school and at home. The device and its charger are property of the Lehighton Area School District.  These items are registered and issued to the student.  The equipment is expected to be well cared for while in the student's possession, and MUST be returned in working condition.  Any wear to these items outside of what is considered reasonable will subject the student to disciplinary action and/or financial restitution sufficient to repair the damage(s) or replacement.

Parents are required to complete the liability statement that can be found in the yearly returning student information submission in order to be able to use the device in and out of school. This form outlines Guidelines for Use, Rules and Responsibilities, and Consequences of misuse. This is to be submitted each year.

Because students and their families are responsible for any wear outside of what is considered to be reasonable, we offer device insurance. Device insurance is available for purchase at the start of each school year or upon enrollment for new students. An electronic copy of the insurance form can be accessed from the link below. If purchasing insurance, it would then be paid online. If a paper copy of the form is desired for payment by cash or check, please ask your school office for one.
