Medication Authorization Form

Medication Administration Policy

Before any medication is admistered to or by any student during school hours, there must be written consent by the parent/guardian on file in the health room giving permission for the administration.  Written consent form will include prescribing physician, the purpose of the medication, dosage, time at which or under special circumstances that the medication should be administered, and length of period which the medication is prescribed.

Students are permitted to self administer the prescribed medication used to treat asthma with the appropriate parent authorization.  Permission to self carry inhalers will be given on a case by case basis.  A written consent as described above needs to be obtained and will remain on file in the health room

*Due to COVID 19, current clinical guidence by the Department of Health prohibits the school from administering nebulizer treatments due to potential aerosol generation, which can more easily spread the COVID 19 virus.  The recommendtioan is to switch to a meter dosed inhaler (MDI) or a dry powder inhaler (turbuhaler or disckus).  Also due to COVID 19, all inhalers must be administered with a chamber to decrease aerosol generation and the spread of the virus.
