REGISTRATION FORMS

To register your child in school please print out the Required Documents Form which includes  Registration, Home Language, Prior Treatment, Sworn Affidavit and Health History forms. 

The following documents are needed to complete registration:

· Proof of Identification Student (Birth Certificate, Passport, etc.)

· Proof of Identification Parent (Driver License)

· Proof of Residence (Property Deed or Rental Lease AND 2 of the following current bills:  Utility Bill, Credit Card Bill, Bank statement, Mortgage statement, Vehicle Registration, Automobile Insurance Card, Property Tax Bill, Health Insurance Card, Sworn Affidavit of Residency, Welfare Card

· Moving Permit

· Social Security Card (optional)

· Proof of Immunization

Helpful documents:

· Current report card

· Current IEP, NOREP, ER

· Custody, Guardianship, Foster Care Paperwork

Other forms available as needed:

Sworn Statement by Resident Under §13-1302 to be Completed by Resident

            To be filled out if you have guardianship of the student

Certification of  Multiple Occupancy by Owner

            To be filled out if you are living with another person and have no proof of Residency

Free and Reduced Price School Meals Application

            To be filled out if you meet the state’s criteria
